Value-based health care in diabetes care

This is a summary of the pilot study regarding the development of a value-based monitoring platform for diabetes care in Sweden.

Initial analysis of data

To understand the relevant outcomes, historical data from 2010 to 2011 on 660,000 patients with type-1 and type-2 diabetes as the main or secondary diagnose was analyzed. The analyses included the initial treatment immediately after diagnosis and the ongoing care, and included data from both the participating county councils’ patient registries and national quality registries.

Value-based health care model

Based on the analysis of the historical cohort and the expert working group the following parameters were included in the VBHC model:

Health outcomes

  • Lower HbA1c
  • Lower predicted 5-year risk
  • Proportion in inpatient care without cardiovascular disease
  • Higher eGFR

Resource use

  • Doctor's visits
  • Net sick-leave days
  • Number of classes of blood pressure medications
  • Proportion treated with statins

The analysis showed statistically significant differences between the county councils in health outcomes and resource use in Swedish diabetes care.

Recommended actions

Based on the analysis, Sveus’ expert group in diabetes care recommended the establishment of a value-based monitoring platform to enable new and important follow-up of diabetes care. In the platform, resource and performance measures can be compared between care providers and county councils.

Sveus is an ongoing research and development project aimed at developing models for value-based health care (VBHC) in several disease areas in Sweden. Seven county councils and regions participate in the work, with support from the Ministry of Social Affairs, including involvement of more than 50 organizations.
